| There are two ways of signing up for MyMedicalFolder: |
| Option 1: Get a username and password |
| If you are in a hurry and have no medical information to enter at this point.You should now proceed to the Quick Registration. To enroll you need to do is fill-in the 6 required fields (those in bold). |
| Option 2: Get a username and password, plus enter background and medical information |
| Enroll for MyMedicalFolder and enter background and medical information at the same time. To enroll you need to do is fill-in the 6 required fields (those in bold). You can also fill in the background information and medical history sections, so that you can start using MyMedicalFolder immediately. To sign up now, you should proceed to the Quick RegistrationPlus. |

| Depending upon how much time you have available, you may want to enter in as much information as you can. Entering data at this stage will automatically be incorporated into the appropriate Tabs, which will reduce time for you in the future. |
| After you have completed at least the required fields, you can click on "Register Now" at the bottom of the screen. |
| A new screen will come up with your Username and Password. PRINT THIS SCREEN or write down the Username and Password. Save it for future reference. |
| Now you are ready to see and use “My Medical Folder.” Click on HOME at the top of the screen. This will bring up a Login screen. Type in your new Username and Password. These are case sensitive, so be sure to type it exactly as it appears on your printout. Also, make sure that the Product field says “My Medical Folder.” Then click Login and you’re ready to start reviewing and adding your medical information to your folder. |
