| A Clinical Call-Center Service Bureau is available to you through a toll-free number, 1-877-298-6108. The Bureau is staffed 24-hours, 7-days a week with licensed clinical care management nurses who are available to assist you in organizing and entering clinical information relative to chronic illnesses, medications and medical procedures. |

| This service includes two additional update contacts with the Service Bureau in the course of the year at no additional charge. |

| Physician Communication. |

| With your permission, Patient Power initiates an e-mail to your primary care physician to review the information entered in “My Medical Folder” and make recommendations for additions or changes. |

| In addition, with your permission, your primary physician will be given access to "My Medical Folder" which can enhance continuity, coordination and the quality of your care. |

| We will also provide your primary care physician with Patient Power software, at no additional cost, to facilitate ongoing updating of your folder. |

| Patient Power Electronic Record Carrier (PERC) is a portable, easy to carry card that can be popped into a computer to immediately access and read your medical information. The PERC serves as a primary medical data source in environments where computer network connectivity to a database is not readily available. There are currently different card carrier options. We are recommending a business card, wallet size mini-CD-ROM. It is secure with password protection.. |

| The charge for Level Two is $25 for the Call Center and the Physician Communication. As an option, there is an additional charge of $10 for a CD-ROM PERC. Additional cards can be ordered for $5 each. An enrollee may want a new card after significant data updates. |
