To sign up for MyMedicalFolder, simply fill out the required fields on form below and
press the "REGISTER NOW" button at the bottom of this page. You will be assigned a username and password
that you can then use to start adding your medical information
to your personal area.

is a little longer, but will allow you to enter background information, family history, any conditions
you might have, and any prescription drugs you might be taking. It is an easy way to initially get
some information into your personal area and register at the same time.
Be sure to write down or print out your username and password when they are displayed.
And please remember that the Username and password are case sensitive, so
write them down accurately and enter them into the login form exactly as they appear.
